Clinic Medical Records

Our Medical Records department handles all incoming and outgoing records on each patient.  Contact our medical records department regarding inquiries and to complete an authorization for release of information form. Southwestern Medical Clinic follows HIPAA regulations regarding patient’s confidentiality. We are unable to fax any medical records unless it involves a life threatening condition.

There is a copy fee of $15 for the first 20 pages and $ .25 for each page thereafter. There is no fee if the records are sent to another physician’s office.

Because our records are not copied on site, it will take approximately 7-10 days for them to be copied and sent back to us.

Financial Policy
  • Each patient is to present their current insurance card at every visit.
  • Southwestern Medical Clinic participates with many insurance companies.
  • Co-payments and deductibles are due at the time of service.
  • Payment for private pay patients is due at the time of service.
  • Payment arrangements may be made with the patient accounts representative.
  • Worker’s compensation cases require written or verbal authorization prior to treatment.

Geriatrics

SWMC provides geriatric specialist care for patients living in long term-care facilities throughout Berrien County. Geriatricians have extensive experience caring for the special needs of an aging population. Physicians and Nurse Practitioners examine patients, order tests and therapies, monitor medication, answer questions, and respond to calls from the nursing home staff.
